Family Care Outpatient

Upon acceptance into the program, you will be expected to complete each phase to attain your certificate. Therefore, you are expected to agree to each of the following:
- You will be placed into an eight-week (8) training/counseling program.
- You will be placed into a mentor program.
- You will be evaluated every four (4) weeks.
- You are required to complete personal studies assigned by your mentor.
- You may be required to attend group class on Wednesday nights.

We have established guidelines that we expect our students to follow while enrolled in the program. These have proven instrumental in helping others overcome life-controlling problems from their past and develop a new, complete change in lifestyle. If at any time we determine you do not follow the guidelines explained in your program, you may be dismissed. If you no call, no show three (3) times, we may discontinue provided services.

You will be asked to pay your $50 candor bond upon your acceptance as a token of your commitment to this program.

Thank you for completing this application. The information supplied will be used to aid in determining the proper direction for help with your life-controlling problem. We want to help you, so the sooner you submit this application, the sooner we can begin the process.

This certifies that I am voluntarily reaching out to Family Care Ministries for training and/or services. For and in consideration of the training and services given or to be given to me while I am a student or trainee with Family Care Ministries, I do hereby waive any and all rights of action and claims of liability against the officers, directors, and staff of Family Care Ministries, severally and individually, as a result of the training and/or services I will receive.
